Application Form for the Milton Young Musicians Festival

We offer three means of obtaining the form which must be completed in order to participate in the Milton Young Musicians Festival:

1. The Milton Public Library’s Reference Desk (Canton Avenue) will have pre–printed copies of the Application Form.
2.   Clicking this link should open the Application Form in your browser (in PDF format). You can print it directly from your computer.
3. Clicking this link should prompt your browser to download a .zip compressed archive of the Application Form to your computer (again in PDF format); you can open and print it at your convenience. (Depending on how your computer is set up, you might try right–clicking (PC) or control–clicking (Mac) the link, and choosing “download linked file to disk” from the pop–up menu.)

Whichever strategy you employ, please remember to complete all portions of the Application Form, include the Registration Fee with the Application Form, and make sure it gets to Mrs. Moulton by the deadline (April 12, 2010):
