Milton Young Musicians’ Festival Application Form

We offer four means of obtaining the form necessary to participate in the Milton Young Musicians Festival:

Due to the 2008 renovation of the Milton Public Library Main Branch, pre–printed copies of the Application Form will be available at the Milton Public Library Kidder Branch (Blue Hills Parkway) after January 23, 2008.

Clicking this link should prompt your browser to download a .zip compressed archive of the Application Form to your hard drive (again in Adobe Acrobat™ format); you can open and print it at your convenience. (Depending on how your computer is set up, you might try right–clicking (PC) or control–clicking (Mac) the link, and choosing “download linked file to disk” from the pop–up menu.)
Failing 1–3, clicking this link will take you to a page where the Application Form has been approximated. The information is the same, it’s just not as attractive

Whichever strategy you employ, please remember to complete all portions of the Application Form, include the Registration Fee with the Applicaiton Form, and make sure it gets to Mrs. Moulton by the deadline (April 15, 2008):
